Try both of the following to access the status of the five locks on your phone;

Go to - All > Device > Advanced System Settings > Sim Card > Identity & Associations.

Go to - Menu > Options > Advanced Options > Sim Card and type MEPD or MEPPD (you will not see any typing as you type).
